Forms & Enrollment

The following forms are required for enrollment.  Please click on the form name to download the form.  Fill out the forms and mail the completed package to the address below.  After downloading the forms, please go to the bottom if this page and fill in the blanks for your family information.  Both parts of this page are necessary for enrollment.

PART 1: FORMS

Please download the appropriate forms and fill them out.  You may print the form and fill it in by hand, but it is preferable that you type the information into the form on your computer.  This will make the forms more neat and readable.  They are on this website as MS Word Documents, but if you cannot open MS Word Documents, contact us and we will email you PDF files.  High School Course Descriptions are available to download in 3 formats - use the one you are most comfortable with.

HSLDA Membership application - new member (Fill this out and mail it to CLA with the above forms and a check for $95 made out to HSLDA.)

HSLDA group transfer application (For existing HSLDA members, fill out this form and mail it with the above paperwork to CLA.  Leave Group Number boxes blank.  If you need to renew, you can include a check for $95 made out to HSLDA which will be forwarded by CLA.)

    Home School Legal Defense Association (HSLDA) membership is required for

    membership in Christian Liberty Academy.  If you are not already a member, you

    may join on their website at www.HSLDA.org  When you enroll with HSLDA, give

    them our Discount Group Number:             This will give you your membership

    for $95 per year rather than the standard rate of $115.
